Quality Assurance Reports
AAQEP offers a variety of resources and guidance to support you in writing the self-study report known as the Quality Assurance Report, or QAR, in application for 7-year accreditation or reaccreditation. This page provides downloadable templates as well as samples excerpted from actual QARs. See Sections 7 and 8 of the Guide to AAQEP Accreditation for detailed guidance on the report's architecture, data considerations, appendices, and more.
Templates and Guidance
-
Program Specification Table (PDF)
This table must be included in the introduction to the QAR as an official record of the programs included in the scope of your AAQEP review. It identifies the degrees or programs being accredited by AAQEP, any associated state credentials, enrollment and completer numbers, and other information. Download this Word version to use as your template. -
Aspect-Evidence Tables (PDF)
These tables, which replace the "Aspect-Evidence Index" introduced in the 2021 Guide to AAQEP Accreditation, provide an organizer for your report readers by identifying evidence sources for Standards 1 and 2, their alignment to the standards and aspects, and which program(s) they support. Download this Word version to use as your template. -

QAR Section Samples
AAQEP members often request samples of other programs’ QARs to help guide their own writing. The passages compiled below offer a sampling of the approaches taken by report authors. Accompanying each excerpt are staff comments highlighting helpful features of the material. Thanks to the faculty and programs who put AAQEP’s spirit of collaboration into practice by agreeing to share these excerpts with other members!
Click on the heading below to navigate to that section’s available samples. Note the downloads are available only to logged-in AAQEP members.

Sending in Your QARWhen: The QAR is due at least 6 months before your site visit, or at least 9 months ahead to take advantage of the staff completeness check. Where: Unless you built your QAR in the Weave Accreditation Management Platform, you'll send your report via email to Sara Hiller, Director of Accreditation Services. Format: AAQEP is generally flexible regarding the format of the QAR (see Sections 7 and 8 of the Guide to AAQEP Accreditation for details), as long as the full report is available for reviewers to access digitally as a cohesive package. Note that the Accreditation Commission will need a PDF version of the report, so you may need to reformat or export your QAR if it is not already a PDF. |
